Admissions Procedures

  • For children over three (3) years of age, notify your local Special Education Director of your desire to have an assessment at the Indiana School for the Deaf. The name and telephone number of your area Special Education Director can be obtained through the Outreach Department at the Indiana School for the Deaf. 800-722-6166 (TTY/Voice), or 317-920-6311 (TTY/Voice).

  • For all children ages 3-21, call Pam Burchett-317-924-8416 (TTY/Voice) for an appointment for an assessment.  Have previous education records, including most recent IEP and the most recent evaluations, sent to ISD, Outreach Department or FAX to ISD at 317-923-2853.

  • For children ages birth-3 years old enrolling in the Early Intervention Program, call Michelle Coleman at 800-724-9550 (TTY/Voice) or 317-924-8415 or email mcoleman@isd.k12.in.us.  Email Jodee Crace at jcrace@isd.k12.in.us

  • The following evaluations will be conducted during the assessment.
    1. Audiological testing
    2. Academic strengths and weaknesses
    3. Social and developmental history
    4. Standardized multifactored test of learning capability (IQ test)
    5. Other testing as requested or deemed necessary to determine educational needs.
    6. Completed otologic form (must be completed by an Ear, Nose, and Throat Doctor).
    Evaluations 1 through 5 may be determined unnecessary by the ISD multidisciplinary team if previous testing is felt to be satisfactory. The above evaluation components are in compliance with Article 7 requirements.

  • Following the completion of the assessment, a letter will be sent to the local school asking that a case conference be called to discuss the evaluation results.

  • If the decision of the Case Conference Committee is to place the student at the ISD, the parent will need to submit the following information in order to complete the enrollment process:
    1. Birth Certificate
    2. Custody papers, if applicable
    3. Registration Form
    4. Medical Form including an immunization record as required by Indiana law
    5. Bus transportation information
    6. Permission to Release for Transportation Form
    7. Permission for Field Trips Form
    8. Permission to Photograph, Videotape and Film Form
    9. ISD Internet Use Policy Agreement
    Forms 3 through 9 are included in an enrollment packet you will receive from the ISD.

    ALL information MUST BE RECEIVED by ISD before the child can begin attending school.

These procedures are in accordance with Indiana Education and Special Education rules and regulations.
